During the Valentine's Day event, a blue tank engine with the body of Thomas and the face of Ralph Wiggum from The Simpsons, known as "Tommy Choo Choo" was available as a manager in the game. This could also be a reference to the 'I Choo Choo Choose You' card Ralph gets in the episode 'I Love Lisa'.

In the third level, The Train That Could, the player is assigned to protect a train pulled by a locomotive resembling Thomas.
Choo-Choo Charles[]
A 2022 indie horror video game developed and published by Two Star Games. The game is based on Thomas the Tank Engine and a book titled Charlie the Choo-Choo. Two Star Games wanted to base a horror game on Thomas the Tank Engine because other horror games did not really delve into parodying children's franchises. Charles' face design bears a resemblance to those of engine characters in the show. (YouTube)

The description of the Toy Train item jokes that a pale face would have been added on the front, but it was decided against due to copyright, a reference to the grey faces seen on the engines throughout the franchise.
Magical Truck Adventure[]
In the first level, Roy's Hometown, a number of engines and buildings resembling those from Thomas & Friends appear. These include engines resembling Thomas, Percy and Toby, as well as one engine with the same colour scheme as James, along with buildings resembling Tidmouth Sheds and the Post Windmill, and a bridge area similar to the Valley Bridge from the third series.
Road Trip Adventure[]
A faceless Thomas toy hauling a carriage that resembles either Annie or Clarabel is one of many decorations that can be added to the player's house.

Street Fighter V[]
A tank engine resembling Thomas appears in the background of the Union Station stage. For copyright reasons, the engine has no face and wears the number 5 where Thomas' number 1 would be.
Tiny Rails[]
A tank engine simply named "Friendly Face Engine" is available as a playable engine in the game. For copyright reasons, the engine has no visible face despite its name, the letter T where Thomas' number 1 would be, and is depicted as a 4-6-0 instead of an 0-6-0.
Train Sim Lite[]
The "Kids Scene" map of the game has a layout based on the Wooden Railway range. Faceless and numberless versions of Thomas, Henry, Gordon, James, Percy and Dennis, as well as a yellow engine repainted from Thomas (presumably Stepney, but using side printing from Oliver), are playable. Additionally, the engines can pull two express coaches or a red breakdown crane. These models were downloadable from TurboSquid. The achievement received from playing the map is called 'Short Stumpy Funnel', a reference to Thomas from the first series episode, Thomas and Gordon.
